Structured residential treatment for adults with substance use disorders and co-occurring mental health conditions, in safe, home-like Minnesota settings.
Residential care that has time to work
ASAM levels 3.1 and 3.5, with programs of up to 90 days.
Many residential programs stop at 30 days. Transformation House programs run up to 90 days, depending on individual need, so there is time to stabilize, build skills, and plan the next step with care.
Residential treatment is often the right fit for moderate to severe substance use, for people with co-occurring mental health needs, and for anyone who doesn't have a safe, supportive home environment right now.

What treatment includes
Evidence-based, strengths-based, and built around the whole person.
Comprehensive assessment
Every stay begins with a diagnostic assessment by licensed counselors and certified mental health professionals, identifying substance use and co-occurring needs.
Individual & group therapy
Daily structured programming, individual counseling, and peer groups addressing trauma, substance use, and mental health together.
Psychiatric care & MAT
Psychiatric support and medication-assisted treatment, including Suboxone, to reduce cravings and withdrawal and lower overdose risk.
Family involvement
Weekly family visits, family group counseling, and education on supporting a loved one through recovery.
Whole-person support
Recreational therapy, mindfulness practices, spiritual counseling, and peer support.
A plan for what's next
Discharge planning with a warm handoff to Recovery Hub outpatient care, sober living referrals, and community meetings.
