Who We Are
A Minnesota behavioral health system with more than 35 years of experience.
We started with a single residential program in Anoka and grew into a coordinated system serving adults from every county in Minnesota. Today, three specialized organizations — Transformation House, Recovery Hub, and Agape Health & Wellness — each focus on what they do best while sharing one clinical standard.
That structure matters to the people we serve. Someone who starts in residential treatment can step down to outpatient care and continue with a psychiatrist or therapist, without repeating paperwork, re-telling their story, or losing momentum.

Our programs are accredited by The Joint Commission and carry its Gold Seal of Approval — independent validation of national standards for quality and safety. We treat those standards as our floor, not our ceiling.
Day to day, we run on a Daily Management System: daily huddles, visual performance boards, and standard work that surface problems in hours, not quarters. Lean management turns improvement into everyday work, driven by the staff closest to the patient.
