Continuum of care: Mental Health and Psychiatry (Agape Health and Wellness) — psychiatric care, therapy, medication management; Outpatient Recovery (Recovery Hub) — IOP, outpatient, community-based care; Residential Treatment (Transformation House) — 24/7 structured care, co-occurring treatment. Patients move in both directions between all three.

That is why Transformation Behavioral Health works through three specialized organizations instead of one catch-all brand.

Transformation House, Recovery Hub, and Agape Health & Wellness stay focused on what they do best, while sharing one standard of care, one set of values, and one coordinated system. People move to the right level of support as their needs change, without losing continuity.

We run on a Daily Management System: daily huddles, visual performance boards, and standard work that surface problems in hours, not quarters. Lean management turns improvement into everyday work, driven by the staff closest to the patient: shorter waits, smoother transitions, better clinical outcomes.

Our programs are accredited by The Joint Commission and carry its Gold Seal of Approval, independent validation of national standards for quality and safety. We treat those standards as our floor, not our ceiling.
